Guarantee Issue Parameters

Coverage is Guaranteed issue to all participants age 70 and under who are actively at work with no medical or financial questions.  The plan requires participants to enroll in a basic benefit selected by their employer. During enrollment, the participant may choose to increase their benefit up to the GI maximum or elect above this amount by answering two additional qualifying questions.  Spouses may also elect coverage by answering 5 qualifying questions.
